CONSENT AND RELEASE FOR USE OF LIKENESS
Effective as of the submission date of this form, approval for past use and permission for present and future use is being granted to Vazhay to use all images, audio and complete likeness of the above named, individual, group or company. Permission is being given by all parties listed, as more fully explained in this Consent and Release. All parties are adults and fully authorized to complete this Consent and Release.
No compensation of any kind is expected by any party for the use of their likeness, all parties hereby grants, to Eppec its agents, employees, licensees, and successors in interest (collectively, the “Released Party”) all ownership rights and the absolute and irrevocable right and permission to copyright, use and publish their photographs and audio likeness that has been (or is being) obtained pursuant to this Consent and Release.
The Likeness may be copyrighted, used, and/or published individually or in conjunction with other photography or video works, and in any medium (including without limitation, print publications, public broadcast, CD-ROM format) and for any lawful purpose, including without limitation, trade, exhibition, illustration, promotion, publicity, advertising, and electronic publication.
All parties represent and warrants that (i) no other party has been granted an exclusive license with respect to the Likeness, and (ii) no other party’s authorization or consent is required with respect to the permission granted to the Released Party under this Consent and Release.
All parties waive any right to inspect or approve the Released Party’s use of the Likeness, or the advertising copy or printed matter that may be used in connection with the use and/or publication of the Likeness. All parties release the Released Party (and all persons acting under its permission or authority) from all claims for libel, slander, invasion of privacy, infringement of copyright or right of publicity, or any other claim related to the Likeness (collectively, “Claims”). This release includes without limitation any Claims related to blurring, distortion, alteration, optical illusion, digital alteration, use in composite form, whether intentional or otherwise, or use of a fictitious name, that may occur or be produced in the processing or publication of the Likeness.

GENERAL INFORMATION

Sponsors are responsible for disposing of all trash in the nearest trash receptacle. We ask that you leave your assigned area in the same or better condition than when you arrived for set-up.

In regards, to power/water, all vendors must be 100% self-contained.

Sponsors assume responsibility for the property they bring to the event and will not hold Vazhay or the event organizer’s, liable for any damages, theft, or personal injury that may occur. Food Truck vendors agree to carry the minimum liability insurance required by the state. Sponsors agree to allow Utrius and the event organizers to use their name/business name and photos, voice and likeness to be used to promote all events present and future.
VENDOR REGISTRATION RULES
SETUP MUST BE COMPLETED 1 HOUR PRIOR TO EVENT START TIME

Vendors should arrive at your entrance location to set up no earlier than 3 hours prior to the event start time. 8:00 a.m. and no later than 1 hour prior to the event start time. Your entrance location will be sent by e-mail to you the week before the event. All vendor space location will be designated upon arrival.

Vendors need to unload their booth contents and remove their vehicle from the area as soon as possible. Bring extra help. Use hand carts/rolling carts.

Vendors are responsible for supplying their own tables, chairs and canopies. Tables should be no longer than 8 feet long. All booth contents must fit within your allowed space.

Food Truck Vendors MUST serve ONE QUICK SERVE “Sample” item that is a “taste portion” for 1-2 people and is listed on your menu for judging purposes.

Menus are required to be posted on the truck with food item prices and accepted payment methods clearly displayed. Attendees will pay vendor directly for any purchases.

Vendors may sell non-alcoholic beverages.

EARLY TEAR-DOWN IS NOT PERMITTED. IF YOU TEAR-DOWN EARLY, YOU WILL NOT BE ASKED BACK NEXT YEAR.

IF YOU HAVE A TRAILOR, YOU ARE ASKED TO SET UP TRAILOR DETACHED FROM TOWING VEHICLE DURING THE EVENT.

Food Truck Vendors MUST dispose of all fats and oils in proper containers and be responsible for removing these containers completely from the site. NO GREASE CAN BE POURED DOWN PUBLIC DRAINS. Disposal of waste in this manner can be subject to a city fine.
